There are times when an individual, court, employer, attorney, or organization may require a formal behavioral health evaluation or written recommendation.

Realign Counseling & Consulting provides a variety of clinical evaluations designed to assess mental health, behavioral health, substance use concerns, workplace accommodations, and other specialized needs. Depending on the purpose of the evaluation, services may include a clinical interview, review of relevant records, screening tools, recommendations, and written documentation when appropriate.

Comprehensive Mental Health & Behavioral Health Evaluations

Comprehensive assessments that provide an overview of mental health symptoms, functioning, history, and treatment recommendations. These evaluations may be requested for personal, legal, workplace, or professional purposes.

Court-Ordered Behavioral Health Evaluations

Behavioral health evaluations completed to satisfy court, probation, legal, or attorney requests. These evaluations assess current functioning, behavioral health concerns, and provide recommendations when appropriate.

Substance Use Evaluations

Assessments that evaluate substance use history, current concerns, risk factors, and treatment recommendations. These evaluations are commonly requested by courts, employers, probation officers, or attorneys.

Anger Management Evaluations

Behavioral health evaluations completed to satisfy court, probation, legal, or attorney requests. These evaluations assess current functioning, behavioral health concerns, and provide recommendations when appropriate.

Emotional Support Animal (ESA) Evaluations

Clinical assessments to determine whether an Emotional Support Animal may be an appropriate accommodation based on an individual's mental health needs.

Return-to-Work / Mental Health Fitness Assessments

Assessments that help determine an individual's readiness to return to work, resume professional responsibilities, and safely perform job-related duties.

Do I need a referral for an evaluation?

In most cases, a referral is not required. However, some courts, employers, attorneys, schools, healthcare providers, or agencies may request specific documentation or evaluation requirements.

What should I bring to my evaluation?

Depending on the type of evaluation, you may be asked to provide identification, court documents, medical records, military records, workplace documentation, or other relevant records that help inform the assessment process.

Are evaluations the same as therapy?

No. Evaluations are structured assessments completed for a specific purpose, such as court requirements, accommodations, employment readiness, Veteran disability claims, or other documentation needs. An evaluation does not establish an ongoing therapeutic relationship unless separate therapy services are arranged.

Are evaluations confidential?

Information obtained during an evaluation is confidential within the limits of the law and professional ethics. However, some evaluations may require reports or documentation to be shared with courts, attorneys, employers, schools, or other authorized parties with appropriate consent.

Can evaluations be used for court purposes?

Some evaluations may be completed for court-related matters, probation requirements, legal proceedings, or attorney requests. Requirements vary depending on the purpose of the evaluation.

Will I receive a report or written documentation after my evaluation?

Most evaluations include written documentation, recommendations, and/or reports. The specific documentation provided depends on the type of evaluation being requested.

How long does the evaluation process take?

Can I complete my evaluation virtually?

Do you provide evaluations for children and adolescents?

What is a Nexus Evaluation?

A Nexus Evaluation is a comprehensive clinical assessment used to determine whether a diagnosed mental health condition is connected to a Veteran's military service. The evaluation may include a review of military records, medical records, mental health history, a clinical interview, symptom assessment, and relevant research when appropriate.

What is a Nexus Letter?

A Nexus Letter is a professional medical opinion written following a Nexus Evaluation. The letter explains the Veteran's diagnosis, relevant military history, clinical findings, and the medical rationale supporting a connection between the condition and military service.

Will I automatically qualify for an ESA letter, ADA accommodation, Nexus letter, return-to-work clearance, or other recommendation?

No. Evaluations are conducted objectively and recommendations are based on clinical findings, documentation reviewed, and professional judgment. Specific outcomes, accommodations, diagnoses, letters, or recommendations cannot be guaranteed prior to completing the evaluation process.

What happens after I submit an evaluation inquiry?
